How it Works

Any VT-based patients (18+) with

a diagnosis or risk factors for:

  • Diabetes
  • Prediabetes
  • Hypertension

Providers refer to

MyHealthyVT.org

A series of free, self-management workshops proven to help your patients overcome hurdles and take control of their health.

Participants get

  • Evidence-based, skill-building workshops
  • Small group setting online or in-person
  • Free to Vermont adults
  • Inviting & fun format for all abilities
  • Regular, social engagement
  • Led by trained facilitators

Providers just need to make the referral and we’ll take it from there.

What providers and care teams can do:

Determine if your patients qualify for My Healthy Vermont. People should:

  • Be aged 18+ and a VT resident
  • Have a diagnosis of diabetes or hypertension
  • Have a diagnosis of prediabetes or any of its risk factors: low physical activity, high blood pressure, abnormal cholesterol, having a relative with diabetes, and/or a score of 5 or higher on our prediabetes risk quiz.

Other workshops may have different eligibility.
